Scoring categories for the appointment of a hospital consultant were changed to give a 40pc weighting to “suitability” – which led to a person with less knowledge and less experience being appointed to a key post.

An interview panel at Children’s Health Ireland (CHI) decided to change the assessment criteria, so that the character of a person shown at interview would amount to 40pc of all the marks available, instead of just half that.
